Umbrella Hub Arrangement Spotlights

Umbrella hub arrangements (UHAs) connect community-based organizations (CBOs) with health care payment systems to pursue sustainable reimbursement for the National Diabetes Prevention Program (National DPP) lifestyle change program. Each UHA is administered by an umbrella hub organization (UHO), which is the lead organization in the UHA. This page showcases the experiences of several organizations that have undertaken the UHO work. The UHOs, the subsidiary organizations they work with, their business models, the obstacles they encountered, and the paths they take to developing UHAs are diverse. The Spotlights provide details about each UHO, their areas of focus, and provide specifics about their UHA development. The approaches these UHOs took to operationalize their UHAs may be instructive to other UHOs, to organizations that might become UHOs, as well as to partners that support UHA development (see the UHA Roadmap page of the Coverage Toolkit for more information on UHA operationalization). The Hawai’i Primary Care Association (HPCA) Spotlight describes the efforts of the HPCA, a Primary Care Association in Hawaii, to plan, build, and launch a UHA as a participant in the Umbrella Hub Demonstration. HPCA has worked diligently with the federally qualified health centers (FQHCs) they serve to leverage the National DPP lifestyle change program as an opportunity to address unmet health related social or economic needs.

The Health Promotion Council (HPC) Spotlight describes the efforts of HPC, a non-profit organization in Pennsylvania, to expand the National DPP lifestyle change program by developing and operationalizing a UHA as a participant in the Umbrella Hub Demonstration. HPC has made significant advancements in developing financial models for the UHA and in creating payer reimbursement pathways.

The Marshall University Spotlight describes the efforts of Marshall University, a West Virgina university, to operationalize a UHA as a participant in the Umbrella Hub Demonstration. Marshall University has leveraged its partnerships in the Appalachian region to strengthen the public health infrastructure and provide pathways to improve the health of the population.

The Trellis Spotlight describes the efforts by Minnesota to support partners in expanding the National DPP lifestyle change program by planning, building, and launching a UHA through their partner organization, Trellis. In addition to providing the National DPP lifestyle change program, Trellis also improves health outcomes in their community by addressing unmet health-related social needs (HRSN) and serving as a community care hub (CCH).
